Fil out the application fully and mail to:

or you can send it to school with your child.
SOME TIPS FOR FILLING OUT THE APPLICATION SO IT IS NOT RETURNED TO YOU FOR CORRECTIONS:
1. Remember to include ALL persons living under the household
roof.
2. Please make sure to use the "family application" as you only need 1 application per household as
opposed to the previous way of filling out 1 per child.
3. Your status from last year will carry over for the first 30 days of school.
4. Please make sure you list ALL income and manner of frequency
(weekly, bi-weekly, etc)
5. Please print legibly and do not forget to sign the form.
6. Do not forget to include your social security number.
7. Do not forget to include the food stamp or OWF number for each child. NOT YOUR MEDICARD CARD
NUMBER!
8. Please fill out your address and phone number completely for faster processing.
We are using the Family Application. This means you need to fill out ONE application to include ALL
of your children in the household.
Remember, you must fill out a new application each year to remain eligible for the free or reduced
meals benefit. To not have a lapse in benefits, you must fill out this form and have it turned in and approved by September
30, 2005.
